300KYA is an independent, privately funded healthcare provider. It does not provide NHS-funded services and does not hold an NHS contract.
DEFINITIONS
In these Terms:
“300KYA”, “we”, “us” and “our” mean Ghassan Mohammed A Iskandrani trading as 300KYA.
“You” and “service user” mean the adult receiving the booked service.
“Partner Laboratory” means Nationwide Pathology or an authorised specialist laboratory involved in analysing, validating or reporting a specimen.
“Booking Platform” means Cal.com or another approved system used to arrange an appointment.
“Appointment” means the agreed home visit for the booked specimen-collection service.
FORMATION OF THE CONTRACT
The description of the selected service, the tests and specimen types stated on the booking page, the total price, these Terms and the booking confirmation together form the agreement between you and 300KYA.
Submitting a booking request does not guarantee an appointment. A contract is formed when:
• the required booking information has been provided;
• the requested service has been accepted as being within scope;
• payment has been successfully received, unless another arrangement has been agreed in writing; and
• 300KYA sends confirmation of the appointment.
Cal.com and any payment processor provide administrative technology. The healthcare-service contract is between you and 300KYA.
The version of these Terms supplied with your booking confirmation will apply to that booking. Any later change will not normally affect an existing confirmed booking unless the change is required by law or safety requirements, or you expressly agree to it.
ELIGIBILITY
The service is available only to adults aged 18 and over.
You may self-refer. A GP referral is not required.
Before proceeding, 300KYA must be satisfied that:
• you are aged 18 or over;
• the requested test and specimen type are within the current service and laboratory scope;
• your identity can be confirmed;
• you can provide valid informed consent;
• the appointment can be conducted safely, privately and professionally; and
• the address is within the published service area.
If another person arranges or pays for your appointment, you must still personally provide clinical consent. The person making payment does not automatically have authority to receive your health information or laboratory report.
WHAT THE SERVICE INCLUDES
Depending on the service selected, 300KYA provides:
• a planned home visit by the Registered Provider, an NMC-registered adult nurse;
• collection of the agreed venous blood specimen and/or instructions for a self-collected urine specimen;
• identity checking, consent and completion of the required clinical and laboratory documentation;
• labelling of specimens in your presence;
• secure packaging, tracking and transport of specimens;
• arrangement of laboratory analysis by the Partner Laboratory;
• verification that the authorised laboratory report matches your identifiers and requested tests;
• secure forwarding of the original, unedited laboratory report; and
• practical signposting to an appropriate healthcare service where required.
The current 300KYA service uses venous blood and/or self-collected urine specimens. 300KYA does not provide swab collection.
The exact tests included in your booking are those expressly listed on the relevant service and booking page. No additional test is included unless confirmed in writing.
WHAT THE SERVICE DOES NOT INCLUDE
Unless expressly stated otherwise, 300KYA does not provide:
• urgent or emergency assessment or treatment;
• diagnosis;
• clinical interpretation of laboratory results;
• prescribing or treatment advice;
• sexual-health counselling;
• partner-notification services;
• ongoing clinical management;
• point-of-care testing;
• imaging;
• laboratory analysis performed by 300KYA itself; or
• tests or specimen types not named in the confirmed booking.
TEST LIMITATIONS
Every laboratory test has limitations.
A result relates only to:
• the infections or conditions named in the booked test;
• the specimen type collected;
• the anatomical site represented by that specimen;
• the laboratory method used; and
• your status at the time the specimen was collected.
A blood and urine screen does not test for every sexually transmitted infection and does not assess every anatomical site. A urine specimen does not assess possible throat or rectal infection.
Tests have window periods. A negative result may not exclude a very recent infection. Testing too soon after an exposure may mean repeat testing is appropriate.
False-positive, false-negative, reactive, inconclusive or indeterminate results are possible. A reactive or abnormal result may require confirmatory testing or further clinical assessment.
The service does not guarantee that an infection is absent or that no future testing will be required.
If you have symptoms, a recent exposure, concerns about the timing of a test, or questions about which specimen is appropriate, seek advice from a GP, NHS or private sexual-health clinic, or another appropriately qualified healthcare professional.
INFORMATION YOU MUST PROVIDE
You must provide accurate and current information required for booking and safe specimen collection, including your:
• full legal name;
• date of birth;
• home-visit address;
• telephone number;
• approved result-delivery contact details; and
• test selected.
You must promptly tell 300KYA if your booking or contact details change.
At the visit, you should disclose relevant safety information such as previous fainting, difficult blood collection, blood-thinning medication, a bleeding condition, or allergies or sensitivities to latex, plasters, tape or skin-cleaning products.
Do not send medical history, symptoms, sexual-health information, identification documents, completed clinical forms or test results through WhatsApp.
IDENTITY AND CLINICAL CONSENT
Before collection or specimen labelling, 300KYA will confirm at least two identifiers, normally your full name and date of birth. Your address or another suitable identifier may also be checked.
Photo identification is not automatically required. However, any discrepancy or reasonable doubt about identity must be resolved before collection proceeds.
You will receive information about the procedure, relevant risks, specimen handling, laboratory testing and result delivery. Immediately before collection, 300KYA will confirm that you understand the service, have capacity to decide and wish to proceed.
You must provide separate written clinical consent at the visit. Agreeing to these Terms online is not clinical consent for blood or urine collection.
You may refuse or withdraw clinical consent at any time. Collection will stop if you ask it to stop. Your right to withdraw consent is not affected by any payment or refund decision. Any refund will be considered separately under these Terms and your statutory rights.
HOME-VISIT REQUIREMENTS, PRIVACY AND SAFETY
The current service is provided at the service user’s private home within the published service area. Do not book a hotel, workplace, office, vehicle, lobby, public area or other community venue unless 300KYA has expressly confirmed in writing that its registered service scope has changed to include that setting.
You must provide:
• lawful access to the address;
• a clean, private and reasonably uncluttered room;
• adequate lighting;
• a stable chair and suitable work surface;
• access to a private toilet and handwashing facilities where urine collection is required;
• a clear and safe exit route;
• a smoke-free environment during the visit; and
• secured pets.
You decide whether a companion remains present, subject to safety, confidentiality and professional-boundary requirements. 300KYA may ask to speak with you privately to confirm consent and confidentiality.
Other people must not interrupt, observe or overhear the procedure unless you have requested their presence and this does not create a safety or confidentiality concern.
Urine collection is completed by you alone in the bathroom or toilet with the door closed. The practitioner does not routinely enter, watch or directly observe urine collection. You will close the container, place it in the supplied packaging, wash your hands and return it as instructed.
300KYA may stop or decline an appointment where privacy, valid consent, infection control or safety cannot be maintained, including aggression, threatening or sexualised conduct, intoxication, uncontrolled animals, unsafe equipment or surroundings, active recording that cannot be resolved, or obstruction of a safe exit.
Any payment or refund decision following an unsuitable or unsafe visit will be assessed fairly according to responsibility for the circumstances, work already performed, unavoidable direct losses and your statutory rights.
BLOOD-COLLECTION PROCEDURE AND RISKS
Blood collection may cause brief pain, bruising, minor bleeding, dizziness or fainting. Infection, significant bleeding or nerve injury is uncommon.
A maximum of two venepuncture attempts is normally permitted during one visit. The procedure may stop earlier if continuing would not be safe or appropriate.
Successful blood collection cannot be guaranteed.
You must follow aftercare instructions and promptly obtain appropriate healthcare assistance if you develop significant or worsening symptoms after the procedure.
PRICES AND PAYMENT
The total price payable for the selected service will be shown before you submit payment.
The booking page will identify what is included, such as the home collection, laboratory tests and any applicable travel or other mandatory charge.
Payment is normally required before an appointment is confirmed.
No repeat-collection fee, additional laboratory fee or other extra cost will be charged without first being explained and agreed.
A confirmed price will not be increased for that appointment unless you request an additional service or agree to a change.
A receipt is available on request.
STATUTORY ONLINE CANCELLATION RIGHTS
Your statutory rights are separate from and take priority over the contractual 24-hour appointment policy below.
Where the Consumer Contracts (Information, Cancellation and Additional Charges) Regulations 2013 apply, you normally have 14 days beginning the day after the contract is made in which to cancel without giving a reason.
You may cancel by making any clear statement that you wish to cancel. You are not required to use a particular cancellation method or the model form at the end of these Terms.
If no part of the service has begun, cancellation during the statutory cancellation period will normally result in a full refund, even where fewer than 24 hours remain before the appointment.
If you request an appointment or other service to begin before the 14-day cancellation period ends, 300KYA will ask you to make a separate express request:
“I expressly request 300KYA to begin providing the service before my 14-day cancellation period ends. I understand that if I cancel after the service has begun, I may have to pay a proportionate amount for services already supplied, and that once the service is fully performed I will lose my statutory right to cancel.”
If you cancel after expressly requesting early performance and after the service has begun, 300KYA may charge only a proportionate amount for services actually supplied before cancellation.
You lose the statutory right to cancel once the service has been fully performed only where you expressly requested early performance and acknowledged that the right would be lost on full performance.
Nothing in this section affects any additional contractual refund offered below.
THE 24-HOUR CANCELLATION AND RESCHEDULING POLICY
In addition to your statutory rights:
• Full refund if you cancel at least 24 hours before your appointment.
• If you cancel with less than 24 hours’ notice after any applicable statutory cooling-off period has ended, the booking may not be fully refundable. Any amount retained will be limited to reasonable direct losses caused by the cancellation that 300KYA could not reasonably avoid. Any remaining balance will be refunded.
• Exceptional circumstances will be considered fairly. Any exception decision and the reasons for it will be documented by 300KYA. Where supporting information is reasonably required, only proportionate information will be requested. Do not send medical evidence through WhatsApp.
• The same 24-hour notice period applies to rescheduling.
• A request received at least 24 hours before the appointment may be moved to another available date without a cancellation charge. Any difference in the price of the replacement service will be explained and agreed before rebooking.
• A rescheduling request made with less than 24 hours’ notice may be treated as a late cancellation and new booking. Any amount retained or additional charge must reflect reasonable unavoidable losses and will be explained.
The notice period is calculated from the scheduled appointment start time to the time 300KYA receives your cancellation or rescheduling notice.
MISSED APPOINTMENTS, REFUSED ACCESS AND WITHDRAWAL AFTER ARRIVAL
If you are unavailable, cannot be contacted, provide an incorrect address, refuse access after arrival or otherwise prevent the booked visit from proceeding, the appointment will normally be treated as a late cancellation.
Subject to your statutory rights, 300KYA may deduct reasonable direct losses that could not reasonably be avoided. Exceptional circumstances will be considered fairly.
You remain free to refuse or withdraw clinical consent. A payment or refund decision will never be used to pressure you to undergo collection.
CANCELLATION OR DISRUPTION BY 300KYA
300KYA may need to cancel or rearrange an appointment because of practitioner illness, severe weather, vehicle breakdown, laboratory disruption, safety concerns or another circumstance preventing safe delivery.
If 300KYA cancels and you are not responsible for the cancellation, you may choose:
• a full refund; or
• a replacement appointment at no additional cost.
If the service has been partly performed, any proposed alternative or refund will be considered fairly according to the service already supplied and your statutory rights.
UNSUCCESSFUL BLOOD COLLECTION
If an adequate blood specimen cannot be obtained despite appropriate attempts, you will receive a full refund. Alternatively, at our sole discretion, you may be offered one (1) optional repeat appointment at a 50% discount on the standard collection fee instead of a refund.
Any laboratory fee or other cost associated with a new test will be explained and agreed before you rebook.
The optional repeat appointment is not guaranteed where another service would be clinically or practically more appropriate.
INABILITY TO PROVIDE A URINE SPECIMEN
The automatic unsuccessful-blood-collection refund above does not apply to an inability to provide a urine specimen.
If you cannot provide an adequate urine specimen, 300KYA will explain the available options. These may include waiting where practicable, rescheduling, or arranging another collection. Any further charge or refund will be explained and assessed fairly according to the service already provided, costs incurred and your statutory rights.
LABORATORY REJECTION OR REPEAT COLLECTION
If the Partner Laboratory rejects a specimen because of a 300KYA collection, labelling, documentation, packaging or transport error, 300KYA will provide a repeat collection without an additional collection charge.
If the repeat cannot reasonably be provided, 300KYA will discuss an appropriate remedy.
If a repeat is required because preparation or collection instructions were not followed, a service-user factor made the specimen unsuitable, or another matter outside 300KYA’s reasonable control applies, a new appointment and payment may be required. The reason and proposed cost will be explained before rebooking.
Once the laboratory accepts a specimen for testing, laboratory costs have normally been incurred and are not normally refundable. This does not remove your statutory rights or any remedy due where 300KYA has not provided the service with reasonable care and skill.
A refund is not provided merely because a result is positive, negative, reactive, abnormal, inconclusive or different from what you expected.
LABORATORY PROCESSING AND RESULT TURNAROUND
The Partner Laboratory is responsible for analysing the specimen, validating the tests and authorising the laboratory report.
Any turnaround time shown is an estimate measured from laboratory receipt, not from collection or dispatch.
Turnaround may be affected by transport, weekends, bank holidays, specimen issues, confirmation testing, external referral testing or laboratory disruption. A turnaround estimate is not a guaranteed deadline.
300KYA will take reasonable steps to monitor expected reports and investigate delays or missing results.
DELIVERY AND USE OF RESULTS
After verifying the identifiers, requested tests and report status, 300KYA will forward the original, unedited laboratory report using the agreed secure method.
Results will not be sent through WhatsApp.
You are responsible for keeping your approved contact details current and for informing 300KYA if you have not received an expected report.
300KYA may explain the result-delivery process and provide signposting but does not provide clinical interpretation, diagnosis or treatment advice.
If the laboratory reports a positive, reactive, abnormal, urgent or otherwise clinically concerning result, 300KYA will try to contact you without avoidable delay and advise you to seek appropriate clinical follow-up.
Sensitive result information will not be left in an unsecured voicemail or disclosed to another person without your authority or another lawful basis.
PRIVACY AND CONFIDENTIALITY
300KYA uses the minimum personal and health information reasonably required to book, provide, document and manage the service.
Booking information may be processed through Cal.com. Necessary information is shared with the Partner Laboratory for specimen analysis and reporting. Other disclosures are made only where authorised or where another lawful basis applies, including safeguarding, legal or regulatory obligations.
Further information about the data collected, lawful bases, recipients, retention periods, your rights and how to contact the ICO is contained in the 300KYA Privacy Notice:

Cookie and similar-technology information is available at:

Acceptance of these Terms is separate from clinical consent and from any optional marketing or cookie consent.
WHATSAPP AND ELECTRONIC COMMUNICATIONS
WhatsApp number: +44 7495 213422
WhatsApp is for booking and general enquiries only.
Please do not send:
• symptoms or sexual-health history;
• medical information;
• photographs of identification;
• completed consent or clinical forms;
• laboratory reports or results; or
• emergency requests.
This WhatsApp number is not for emergencies and must not be relied upon for urgent clinical advice.
COMPLAINTS AND FEEDBACK
You may raise a concern during the visit or afterwards by email, telephone or writing to the contact details in section 1.
Raising a complaint will not disadvantage you or affect the standard of service you receive.
Complaints are normally acknowledged within three working days. A full response is normally provided within 28 days. If more time is required, you will be told why and given a revised timescale.
Information can be provided in another reasonable format, and communication support will be considered where required.
If you remain dissatisfied, you may ask the Registered Provider to review whether the complaint was investigated fairly and whether the response and remedy were appropriate.
You may also give information about the quality or safety of a regulated service to the Care Quality Commission. CQC uses such information in its regulatory work but does not normally resolve an individual complaint on the complainant’s behalf.

URGENT AND EMERGENCY HELP
300KYA is not an emergency or urgent-care service.
If you believe there is an immediate danger or medical emergency, call 999.
For urgent medical advice that is not an emergency, contact NHS 111, your GP, a sexual-health clinic or another appropriate healthcare service.
Do not wait for a WhatsApp, email or booking-platform response if you need urgent help.
REASONABLE CARE, LIABILITY AND CONSUMER RIGHTS
300KYA will provide its services with reasonable care and skill and in accordance with applicable professional and regulatory duties.
Test limitations, window periods and estimated turnaround times are explained to support informed decision-making. They do not remove 300KYA’s duty to provide the service with reasonable care and skill.
Nothing in these Terms excludes or limits liability where it would be unlawful to do so, including liability for:
• death or personal injury caused by negligence;
• fraud or fraudulent misrepresentation;
• breach of duties that cannot lawfully be excluded; or
• your statutory consumer rights.
Nothing in these Terms makes you responsible for an error caused by 300KYA.
300KYA’s use of a Partner Laboratory does not remove any responsibility that cannot lawfully be transferred or excluded.
300KYA will not be responsible for a delay caused by circumstances genuinely outside its reasonable control where it has taken reasonable steps to prevent or reduce the effect. This does not remove your right to an appropriate refund or remedy for services not supplied.
CHANGES TO A CONFIRMED SERVICE
If a material change becomes necessary after booking, such as a change to the tests, specimen type, price, appointment arrangements or Partner Laboratory, 300KYA will explain the change before proceeding.
Where required, your express agreement will be obtained. If you do not accept a material change, you may cancel and receive an appropriate refund for services not provided.
ACCESSIBILITY AND REASONABLE ADJUSTMENTS
Please tell 300KYA if you require information in another format, communication support, additional explanation, extra time or another reasonable adjustment.
Requests will be considered individually according to your needs, safety and the scope of the service.
GOVERNING LAW
These Terms and the contract are governed by the law of England and Wales.
The courts of England and Wales will have jurisdiction, subject to any mandatory consumer right allowing you to bring proceedings in another part of the United Kingdom.
GENERAL TERMS
If a court finds that part of these Terms is unlawful or unenforceable, the remaining terms will continue to apply.
A delay by either party in enforcing a right does not automatically waive that right.
No person other than you and 300KYA may enforce these Terms, except where the law provides otherwise.
